• School, camp, and sports forms promptly available at no extra cost.

• Through partnership with My Concierge Pharmacy, enjoy home deliveries of prescription medications and direct access to the pharmacist for a monthly consultation on your child’s routine medications or supplements.

• Age 0-23 months: $450/month ($5,400/year)

• Age 2 -5 years: $370/month ($4,440/year)

• Age 6-18 years: $325/month ($3,900/year)

There will be a one-time family registration fee of $500 paid at the time of initial enrollment or any subsequent re-enrollment after cancellation. This registration fee is non-refundable.

A 5% discount is available if the membership fee is paid in full at the time of enrollment. For families with three or more children, we offer a 20% discount on the fee for the youngest child.

Membership cancellation will require a minimum 30-day notice. If membership fees were pre-paid in full annually, the prorated value of the unused months of service will be refunded within 30 days of cancellation.

• Exclusive package including comprehensive newborn care in the first 60 days of life for $1,650. A non-refundable registration fee of $500 is due at the time of initial enrollment.

• Three house visits for the first 3 wellness appointments at 3-5 days, 2 weeks, and 1 month. 2 month visit not included. 

• Direct phone call or text message correspondence with the doctor.

• After day 60, we would love to continue being your pediatrician after signing up for a full membership. The cost of the newborn package will be discounted from your yearly membership fee. However, you can choose to transition care to another provider.

• Non-members can schedule a virtual appointment for a sick visit. During the consultation, concerns are evaluated and we can determine if a house-call is warranted or if care must be escalated to an emergency room. Fee is $150.

• If a house call is needed, the fee is $500, and the virtual consultation fee is waived. This covers all point-of-care diagnostic testing and any necessary medication administration.

• Additional children seen in the same house-call incur a fee of $250.

• Text message access to the physician for 48 hours after the visit for follow up questions or concerns.

• To ensure continuity of care, the visit documentation will be e-faxed to your pediatrician.

• Non-members can schedule a house-call visit for a physical exam required for sports participation or for school entry. Fee is $500.

• Forms are filled out after the exam and returned the same day or the next business day.

• Routine immunizations required for school entry by the Florida Department of Health must be up-to-date to receive a certified state immunization form. No vaccines will be administered during this visit.

• Additional children seen in the same house-call incur a fee of $250.
