Our services

• Exclusive package including comprehensive newborn care in the first 60 days of life for $1,200.

• 3 house visits for the first 3 wellness appointments (3-5 days, 2 weeks, 1 month). 2 month wellness visit not included. 

• Unrestricted phone call or text message correspondence with the doctor.

• After day 60, families can either sign up for a full membership or transition care to another provider. If you choose the full membership, we'll apply a discount for the newborn package to your yearly membership fee.

• Non-members can schedule a virtual appointment for a sick visit. During the consultation, concerns are evaluated and we can determine if a house-call is warranted or if care must be escalated to an urgent care or emergency room. Fee is $150.

• If a house call is needed, the fee is $500, and the virtual consultation fee will be waived. This fee covers all point-of-care diagnostic testing and any necessary medication administration. Additionally, an e-fax of the visit documentation will be sent to the patient's pediatrician.

• Additional children seen in the same house-call incur a fee of $250.

• Text message access to the physician for 48 hours after the visit for follow up questions or concerns.

• Home visit boundaries within a 10 mile radius of the office applies.

• Scheduled ahead of time, per American Academy of Pediatrics (AAP) recommended intervals to provide utmost preventive care and screening.

• All visits done in the comfort of your own home. Home visit boundaries within a 10 mile radius of the office applies.

• Childhood vaccines.

• School, camp and/or physical forms at no extra cost.

• Unrestricted face-to face, phone or text message consultations with the doctor directly.

• Urgent communication after-hours.

• Unrestricted sick visits through appointment only with same-day or next-day availability.

• 2 in-home sick visits included in your membership. Additional sick visits available at an additional, though discounted, cost of $250.

• In-home wellness visits and childhood vaccines per the AAP schedule.

• A wide range of point-of-care diagnostic and screening tests.

• Many common minor procedures included.

• Age 0-2 years: $416/month ($5,000/year)

• Age 3-11 years: $308/month ($3,900/year)

• Age 12-18 years: $291/month ($3,500/year)

There will be a one-time family registration fee of $500 paid at the time of initial enrollment or any subsequent re-enrollment after cancellation. This registration fee is non-refundable.

There will be a 5% discount if the membership fee is paid in full at the time of enrollment. Discounts are available for families with 3 or more children.

Membership cancellation will require a minimum 30-day notice. If membership fees were pre-paid in full annually, the value for the unused months of service will be refunded.
